Why are we conducting this survey?

The purpose of this study is to better understand the care experiences of individuals with life-limiting illnesses and their unpaid caregivers, including access to care and support services.

The study will provide information on:
  • experiences and satisfaction with the care received to meet physical, psychological, social and spiritual needs,
  • gaps and barriers in care.
The results will help health care organizations and providers improve the development, planning and delivery of high-quality care for those living with a life-limiting illness and their unpaid caregivers in Canada.

This survey is anonymous. No effort will be made to link your responses to other information about you collected from any other surveys or data sources. Please do not enter any identifiable personal information in your responses. If any identifiable personal information is entered, it will be disposed of and not used.

Your information may also be used by Statistics Canada for other statistical and research purposes.

Although voluntary, your participation is important so that the information collected is as accurate and complete as possible.
