Logging In

The 16-character secure access code can be found in the invitation that was sent to you. If you cannot locate your secure access code, or have misplaced your invitation letter, contact us.

There are a number of reasons why your secure access code may not work: it may have been entered incorrectly; the survey collection period may be over; the secure access code may be disabled; or, cookies are disabled in your browser.

Note: The secure access code will not work if the collection period for the survey has ended or if the code has been disabled in our system. Please contact us for assistance.

Check that you entered your password correctly or re-enter it. Make sure that the “Caps lock” key is not on. Enable cookies in your browser.

For security reasons, the session times out after 120 minutes of inactivity. To re-open your session, simply re-enter your secure access code and your password, if applicable.

Based on the answers you provided, some questions may not apply to you and as result you will be navigated through the questionnaire accordingly.
