What is the questionnaire list?

This is a page where respondents can view and access all the questionnaires for a specific survey, which enables them to manage their questionnaires more easily. The questionnaires are grouped by reference period, in tables, which can be collapsed or opened.

The columns in the table include Questionnaire Status, Due Date, Questionnaire ID, Legal Name, Operating Name, and Coverage Statement. In the Questionnaire Status column, questionnaires can be marked “Not Started,” or “In Progress.”

Does everyone have access to the questionnaire list?

Not everyone has access to this feature. Usually, only respondents who have multiple questionnaires to complete for a specific survey will have access to it. If this applies to you but you are not able to view the questionnaire list, you may see it at a later date once the system has been updated.

How do I navigate the questionnaire list?

There are several ways to navigate the questionnaire list:

  • View and collapse tables by selecting the reference period.
  • Show 10, 50, 100 or all questionnaires in the table.
  • Sort the table by clicking the arrows in the column header.
  • Search by keyword.

Note: If there are many questionnaires in your list or if there is heavy online traffic, it may take longer to open the page. If the service is unavailable, a message will appear.

How do I open a questionnaire?

To open a questionnaire, select the hyperlink in the Questionnaire Status column.

How do I return to the questionnaire list from a questionnaire?

If you are logged into a questionnaire, you can return to the list by using one of the following methods:

  • Complete the questionnaire, then press the Submit button. On the Thank-you page, press the Next button.
  • Press the Save and finish later button. You will then be given the option to return to the list.

How do I find out the status of the questionnaires?

You can view the status of the incomplete questionnaires in the Questionnaire Status column. The questionnaires will be marked either “Not Started” or “In Progress.” The deadline for completing the questionnaire is provided in the Due Date column.

Once a questionnaire has been completed and submitted, a confirmation code will be provided on the Thank-you page. Please write it down or print that page.

If the deadline has passed for submitting a questionnaire, an “Overdue” message appears in red in the Due Date column. This indicates that the questionnaire(s) should be completed as soon as possible.

Why are some columns left blank in the tables?

Blank columns in the tables mean that the information was not available when the questionnaire list was created.

However, you can view information about a questionnaire by referring to the Questionnaire ID in the table. This ID remains the same for all reference periods.

How do I log out of the questionnaire list?

From the list, select the Log out hyperlink in the top right corner. If you are in a questionnaire, you can either press the Save and finish later button or select the Log out hyperlink. In both cases, you will be given the option to log out completely.

We recommend logging out of the list at the end of your session.

After two hours of inactivity, your session will time out and you will be logged out automatically.
